Do you have a fever or feel feverish or hot in the last 14 days?  Do you have a cough or difficulty breathing or shortness of breath?  Do you have any flu-like symptoms?  Have you experienced a loss of taste or smell?  Have you been in contact with any confirmed COVID-19 positive people?
